The instructions below tell WorkGroup Leaders and and Volunteers what to do with these forms.  To download a form,  right-click it, and then choose to save to a file on your local hard drive.  Downloaded forms may then be opened in MS Word, filled in, and sent it electronically as a file attachment to an email note.

90 Days before Arrival:  After completing online registration, Work Group Leader submits online version of the CUPS WorkGroup 90 Day Form, and sends in a Work Group deposit of $1,000.

60 Days before Arrival:  Work Group Leader submits online version of the CUPS WorkGroup 60 Day Form, and sends in balance of Work Group fee ($3,300).

30 Days before Arrival:  Work Group Leader submits online version of the CUPS WorkGroup 30 Day Form, which now includes the La Quinta Condel Room List and Work Group Checklist.

  Parental Permission for Sponsor of a Minor

Sponsor (adult volunteer) completes online registration for minor.

  Ø

Sponsor and both parents complete and sign form, and give it to the Leader.

Leader checks form, verifies signatures, keeps original for use in Mexico, and sends a copy to CUPS Houston office.

   Volunteer Release and Insurance Statement

Volunteer first completes online registration.  He can then fill in his Volunteer Release and  Insurance Statement online, print it, sign it, get witness signatures, and submit to workgroup leader.

  Designated Driver Form

Leader must recruit 2 designated drivers for each vehicle going into Mexico.  This form is for his use in obtaining the required information on these drivers.

Leader prints this form for his/her own use.  It covers many of the problems encountered in previous years.

Leaders or volunteers can print these forms for their own use.  They are guides for obtaining paperwork and signatures for permits to take vehicles into Mexico.  Each form is presented in both Spanish and English:
